This Letter of Agency (“LOA”) authorizes the release of all Customer Proprietary Network Information (“CPNI”), as defined in 47 U.S.C. §222, to Intra Systems. Such CPNI includes but is not limited to customer name and number, billing records, service records, and network and equipment records, for the purpose of providing telecommunications or information services. This LOA becomes effective on the signature date and remains in effect unless revoked in writing.
- The parties acknowledge that the customer has obtained CPNI as defined in 47 U.S.C. §222.
- The customer authorizes Intra Systems to use, disclose or access such CPNI as needed for the provision of telecommunications services to the customer’s end users. This includes, but is not limited to, population of CNAM databases by third-party providers.
- The parties acknowledge that, pursuant to 47 C.F.R. §64.2005, the customer may use, disclose or permit access to CPNI for the purpose of providing service without further authorization from its own customers.
- The customer agrees it will not require Intra Systems to use, disclose or access CPNI for any reason other than the provision of telecommunications or information services, as provided in 47 C.F.R. §64.2005(a). Where Intra Systems’ performance under this agreement extends beyond that scope, the customer will provide any necessary written authorization prior to Intra Systems undertaking those activities.
- Intra Systems agrees to take all reasonable steps to protect CPNI provided to it by the customer, in compliance with 47 U.S.C. §222.
